Assessment stage could be reactivated if significant change of circumstances/concerns are notifed by the implementing provider(s).
This might include a request to cease current RP and/or establish RP
Intake
Stage

Complete risk assessment
-
Report unathourised use of Restrictive Practice (RP) or use of prohibited practices within 24hrs
-
Assign Behaviour Support Practioner (primary plan writer) and Behaviour Support Practioner
-
Clinical handover -especially info regarding previous PBSP or behaviour support input, and current RP and behaviour support plan (BSP) details

Attend initial stakeholder meeting(s)

Apply person centred principles ongoing.

If risk shows need for RP then initiate approval with State/Territory ASAP.
Assessment
Stage
(0-1 months from intake)

Engagement of participant’s support network with consent of participant for information and data gathering, including collection of colleteral history.

Conduct Functional Behaviour Assessment (FBA) and develop a FBA report.

If restricted practice is needed, develop an Interim BSP including details of the RP to apply approval from State or territory. Lodge the interim BSP & RP via NDIS Commision Portal after approval from State or Territory.

Liaise with implementing provider(s) & other stake holder for implementing the RP and BSP (if required).

Ongoing data collection

Educate implementing provider(s) on data collection , interim BSP and RP (if required)

If RP is involved, develop Comprehensive BSP and lodge to NDIS Commision via NDIS Commision Portal

Finalise data collection protocol and plan implementation training

Regular communication with implementing provider(s) via stakeholder meeting and individual engagement

Ongoing reporting or RP use. Get participant’s view of RP.
Review and Maintenance
Stage

Initial review of BSP/RP within 1 month post the draft plan is developped

Ongoing behaviour data collection

Develop regular (minimum 12 months) formal BSP review report

Meet BSP report renewal schedule to ensure BSP is renewed and lodged before the expire date

Review of RP with implementing provider(s) to monitor evaluate effectiveness of BSP and RP use and how to reduce or eliminate RP.

Ensure participant and other’s views considered.
